In this webinar, Dr. Kemere will discuss identifying common barriers to a successful transition, recognize opportunities to talk transition early and often, and learn what is helpful for adult providers to know about patients.  Joining Dr. Kemere will be Laura Buckner, M.Ed., LPC/Program Manager, Texas Center for Disability Studies at University of Texas at Austin.  She will address the subject of identifying and discussing the importance healthcare transition planning, including learning how to create a trajectory to help guide you through your health care transition.
